Please complete this form to submit a warranty claim for products purchased from Amplen Industries. Ensure all required fields are filled out accurately. You must provide details for at least one product including its serial number, brand/description, and a photo of the serial number. The original invoice is mandatory for all claims. Additional documentation such as the Certificate of Compliance (COC) and installer details can be provided to expedite your claim process.
